Please fill out the online health history form for your child if you have not already done so. You can call me with any questions or concerns at 254.215.6449
Immunizations! Parents, if you take your child for immunizations at any time during the school year (especially 6th graders Tdap and Meningococcal that are required for 7th grade), please send a copy to Alyssa Danielson at [email protected] so we can update your child’s school record. Thank you!
Students may NOT have any over-the-counter OR prescription medications at school without a doctor note on file in the nursing office. Medications may NOT be carried in their backpack or pockets unless their primary care physician specifically writes "MAY SELF CARRY" for rescue inhalers, EpiPens and Diastat. Campus staff will take the medication and call parent/guardian if medications are found on student.
