Please fill out the online health history form for your child if you have not already done so. You can call me with any questions or concerns at 254.215.6449
Immunizations! Parents, if you take your child for immunizations at any time during the school year (especially 6th graders Tdap and Meningococcal that are required for 7th grade), please send a copy to Tammy Diserens at [email protected] so we can update your child’s school
record. Thank you!
Students may not have ANY over the counter or prescription medications at school without a doctor note on file in the nursing office and may not be carried in their backpack or pocket unless the doctor specifically writes MAY SELF CARRY for inhalers, Epi Pens, and Diastat. We will take the medication and call the parent if we find medications on the student.
